The Nurse Practitioner Cardiogenic Shock Program Manager isresponsible for providing leadership, expertise, collaboration,consultation and mentorship for the team responsible in promotingevidence based practice in the care of the Cardiogenic Shockpatient. The Manager is an expert and patient advocate in themanagement of patients requiring advanced cardiogenic shocktherapies including percutaneous mechanical circulatory support.

This position manages a group of clinically complex or resourceintensive patients and aligns with evidence based medicalbenchmarks (SCAI Shock Initiative, AHA, ACC, STS, i.e.). TheCardiogenic Shock Manager is the Program Leader in a hub and spokemodel supporting enterprise wide strategic and organizational goalsunder the guidance and direction of the Vice President and Medicalleadership of the Cardiovascular Care Transformation Services (CTS)and Shock Program Medical Director in conjunction with campus andprogram Medical Director(s), Physician(s)/ Campus specificAdministration(s) and various CV leaders to achieve optimalstandards of care with measurable identified outcomes. Minimum 6years of experience as an APN. Minimum 5 Years of Experience incardiovascular surgery and interventional cardiology.

Responsibilities 1. Works in conjunction with the VP CardiovascularCare Transformation Services (CTS) and site specific leadershipteams to develop programs that achieve accreditation requirements,quality care delivery, and desired patient experience. Developsposition descriptions for each area, defines workprocesses/protocols, and develops/implements consistent performancestandards for each area across the

Incollaboration with CTS and regional campuses assists in developmentof the strategic planning initiatives for cardiogenic shock andrelated programs (Advanced HF (VAD)) across the Network incollaboration and standardization enterprise wide with the advancedHF team. 2. Responsible for program data management including butnot limited to: formal data tracking and submission for DSCs;development and maintenance of performance dashboards or scorecardsto support management of the spine programs. Track, analyze, andreport on relevant data and metrics as needed and/or requested.

Develop, recommend and/or implement actions or process improvementsas appropriate. 3. Part of the Leadership team responsible for thecreation of the Cardiogenic Shock team initiative. Partners withCardiology, Advanced Heart Failure, Cardiac Surgery and CriticalCare teams in evaluating and treating with mechanical circulatorysupport for the Network. 4. Practices as an autonomous andcollaborative member of the healthcare team, demonstrating andteaching clinical expertise, differentiating between normalfindings and those needing treatment, referral and or consultation.5. Lead process improvement efforts to address gaps and align withevidence based, standardized care for the cardiogenic shockpopulation. 6. Demonstrates advanced assessment skills, withinclinical specialty, and in the development of the cardiogenic shockteam in providing evidence-based treatment interventions, toachieve positive outcomes for patients and families. 7. Coordinatespeople, places and products to provide clinical guidance andspecialty leadership through participation and/or representation atunit, departmental and hospital meetings, committees, councils,task forces, health fairs, educational presentations andmultidisciplinary rounds, as needed. 8. Develops educationalresource materials independently and in collaboration with otherhealth care providers. Functions as a mentor, educational resource,consultant, for staff providing clinical expertise. Serves as aresource and support person for ventricular assist device patientsand their family members both in the acute care setting and in thecommunity. Works in conjunction with Nursing and DepartmentalEducation to ensure clinical competency for all staff caring forcardiogenic shock patients. 9. Participates in the development andrecommendation of evidence-based practice identifying the goal anddesired measurable outcomes, which may include clinical, financial,process/system and patient/family satisfaction. 10. Exploresopportunities to expand the existing scope of services to growrevenue and improve quality care delivery in alignment with theCardiovascular Transformation Team `s strategic vision. Managestime and resources effectively by managing multiple priorities withappropriate delegation. Develops, implements, and supportsactivities and/or projects that promote patient satisfaction withinthe Office of Human Experience framework to achieve best practicetargets. 11. In collaboration with the CV Transformationleadership, assists in the development of cardiogenic shockprotocols and its relation to our enterprise wide marketingcampaigns and community outreach. 12. Educate all staff regardingcare of cardiogenic shock patients in collaboration with thecardiovascular attending of record to prevent unnecessaryreadmissions. 13. Provide preceptorship to the other sites withinthe network. 14. Precept APP students in clinical practicum. 15.Participate at various national and local meetings and representHackensack Meridian Health on the topic of cardiogenic shock. 16.Functions as APN, as needed. 17. Other duties and/or projects asassigned. 18. Adheres to HMH Organizational competencies andstandards of behavior. 19. Lifts a minimum of 25 lbs., pushes andpulls a minimum of 25 lbs. and stands a minimum of 6 hours a day.

Requires ability to travel to HMH Tertiary care hospitals,physician offices, and administrative locations through thenetwork. Requires time to support program events after hours andweekends as needed to promote the programs.

QualificationsEducation, Knowledge, Skills and Abilities Required:

1. Graduate of

NLN/AACN

accredited program in nursing. 2. Masters Degree. 3.Minimum 6 years of experience as an APN. 4. Minimum 5 Years ofExperience in cardiovascular surgery and interventional cardiology.5. Excellent written and verbal communication skills. 6. Proficientcomputer skills that may include but are not limited to MicrosoftOffice and/or Google Suite platforms.

Education, Knowledge, Skillsand Abilities Preferred:

1. Shock experience.

Licenses andCertifications Required:

1. Advance Practice Nurse License. 2. NJState Professional Registered Nurse License. 3. BLS AHA BasicHealth Care Life Support HCP Certification. 4. Advanced CardiacLife Support Certification.
