Growing together. The RN Clinical Care Coordinator will be theprimary care manager for a panel of members with complexmedical/behavioral needs. Care coordination activities will focuson supporting member’s medical, behavioral, and socioeconomic needsto promote appropriate utilization of services and improved qualityof care. Expect to spend about 80% of your time in the fieldvisiting our members in their homes or in long-term carefacilities. You’ll need to be flexible, adaptable and, above all,patient in all types of situations. Do you live or are you willingto travel to any of the following counties?
Sommerset and MorrisCounties, NJ Primary Responsibilities:
- Engage membersface-to-face and/or telephonically to complete a comprehensiveneeds assessment, including assessment of medical, behavioral,functional, cultural, and socioeconomic needs
- Develop andimplement person centered care plans to address needs includingmanagement of chronic health conditions, health promotion andwellness, social determinants of health, medication management andmember safety in alignment with evidence-based guidelines
- Partnerand collaborate with internal care team, providers, and communityresources/partners to implement care plan
- Provide education andcoaching to support member self-management of care needs andlifestyle changes to promote health
- Support proactive dischargeplanning and manage/coordinate Care Transition following ER visit,inpatient or Skilled Nursing Facility (SNF) admission
- Advocatefor members and families as needed to ensure the member’s needs andchoices are fully represented and supported by the health care teamYou’ll be rewarded and recognized for your performance in anenvironment that will challenge you and give you clear direction onwhat it takes to succeed in your role as well as providedevelopment for other roles you may be interested in.
RequiredQualifications:
- Current, unrestricted independent licensure as aRegistered Nurse
- 2 years of clinical experience
- 1 years ofexperience with MS Office, including Word, Excel, and Outlook
- Driver’s license and access to reliable transportation and theability to travel up to 80% of your time within assigned territoryto meet with members and providers
Preferred Qualifications:
- BSN,Master’s degree or higher in clinical field
- CCM certification
- 1years of community case management experience coordinating care forindividuals with complex needs
- Experience working in team-basedcare
- Background in managed care
Physical Requirements:
- Abilityto transition from office to field locations multiple times per day
- Ability to navigate multiple locations/terrains to visitemployees, members and/or providers
- Ability to transportequipment to and from field locations needed for visits (ex.laptop, stethoscope, etc.)
- Ability to remain stationary for longperiods of time to complete computer or tablet work duties Thehourly range for this role is $28.
61 to $56.06 per hour based onfull-time employment. Pay is based on several factors including butnot limited to local labor markets, education, work experience,certifications, etc. UnitedHealth Group complies with all minimumwage laws as applicable.
