We are seeking a positive team member to join our organization of compassionate healthcare professionals who take pride in helping each patient obtain and maintain good health by guiding them on a path to wellness. The Clinical Nurse Manager is responsible for the daily activities of the health center as it relates to patient care and treatment. In this role, you will: Work closely with health center administration and managers to ensure the health center operates in a coordinated, organized manner such that quality care and services are provided. Serve as a member of Quality Assurance, Infection Control, and other committees as deemed necessary. Participate in the development, review, revision, implementation, and monitoring of policies/procedures at the direction of the supervisor. Provide care in a non-judgmental, non-discriminatory manner that is sensitive to the patient’s and family’s diversity, preserving their autonomy, dignity, and rights. Supervise LPNs and other medical staff as necessary in all clinical activities. Manage care for patients with complex medical and behavioral health needs. Develop plan of care individualized for the patient reflecting collaboration with other members of the healthcare team. Track, measure, and report HEDIS to Quality Assurance Committee and Health Plans. Prepare pre-visit planning to improve Quality outcomes for the patient and provider. Participate in daily huddle, identifying specific needs for successful patient encounters. Plan ER/Hospital/Urgent Care Discharge, care coordination, and transition of care. Utilize NYSIIS for immunization tracking/upload. Participate in Chronic Care Management/Population Health Management Programs.

We require:

Associate degree in Nursing required; Bachelor’s degree in Nursing preferred. NYS Registered Nurse Licensure. Excellent verbal and written communication skills. Bilingual (English/Spanish) preferred. Excellent organization skills. Proficient computer skills. Committed and effective team worker. Strong commitment to the provision of quality medical care for medically underserved individuals. Proven capacity to work productively in highly collaborative settings and to seek and synthesize input from multiple stakeholders.
