Family First Homecare New York, NY Job Summary:

The Director of Patient Services (DPS) is a registered nurse responsible for overseeing the clinical operations of the home care agency. The DPS ensures compliance with state and federal regulations, supervises nursing staff, manages patient care plans, and promotes high-quality, person-centered care.

Key Responsibilities:

Supervise all clinical staff, including RNs, LPNs, and HHAs. Develop, implement, and evaluate patient care plans in coordination with physicians and caregivers. Ensure agency compliance with DOH, Medicare/Medicaid, and other regulatory requirements. Conduct regular audits and quality assurance checks on clinical documentation and service delivery. Provide training, guidance, and performance evaluations for clinical team members. Participate in on-call rotation and respond to urgent clinical concerns. Serve as a liaison between patients, families, staff, and medical providers. Maintain current knowledge of home care regulations and best practices. Coordinate case conferences and care planning meetings. Monitor and manage clinical outcomes and implement corrective actions as needed.

Qualifications:

Current RN license in [State] (required). Minimum 2-3 years of supervisory experience in home care or related healthcare setting. Strong knowledge of state and federal home care regulations. Excellent communication, leadership, and organizational skills. Ability to work independently and collaboratively in a fast-paced environment.

Preferred:

Bachelor’s Degree in Nursing (BSN). Prior experience as a DPS or in a similar leadership role within a LHCSA or CHHA.
