Qualifications:

Education:

Associate’s or Bachelor’s degree in Nursing (ADN/BSN).

Licensure/Certifications:

Active New York RN license. MDS Assessor Certification (required).

Experience:

1-2 years in long-term care with MDS coordination experience .

Soft Skills:

Attention to detail, strong communication, and ability to lead interdisciplinary teams.

Key Responsibilities:

Intake and evaluation process of patients in long-term care facilities. Up-to-date assessments of the patient and oversee the interdisciplinary assessment for all nursing home patients. MDS coordinator monitors patient care by assessing procedures. Speaking with patients and recording medical codes.
