  • to join their innovative organization and help shape the delivery of exceptional, patient-centered care across our community programs.
Overview:

As the ACSD, you will play a vital role alongside the Clinical/Center Service Director, supervising daily operations of our interdisciplinary teams and ensuring seamless, high-quality participant care. You will serve as the Administrator for our LHCSA, coordinating integrated care across community settings, Day Health Centers, Diagnostic & Treatment Centers, and beyond.

Responsibilities:
  • Support and oversee site-based managers and clinical staff, including teams in DTC, Day Health Centers, rehab, nutrition, social work, and home services.
  • Lead the coordination of Care Planning meetings, ensuring timely assessments, follow-ups, and mediate team discussions to optimize patient care.
  • Allocate resources effectively across teams, balancing workloads and ensuring optimal coverage.
  • Drive the development and execution of personalized care plans that enhance participant satisfaction and health outcomes.
  • Collaborate with centralized teams to ensure assessments and care plans are completed efficiently and accurately.
  • Oversee service delivery to ensure participants receive the right care, at the right time, and at the right cost-delivering the highest quality standards.
  • Facilitate ongoing staff training, professional development, and implementation of management directives.
  • Monitor operational metrics, participate in quality assurance activities, and ensure compliance with all regulatory requirements.
  • Work closely with clinical development teams to establish innovative care protocols and improve service delivery.
  • Address and resolve any participant grievances or issues swiftly, maintaining a courteous and professional approach.
Qualifications:
  • A dedicated healthcare leader with experience supervising interdisciplinary teams in a clinical setting.
  • Knowledgeable about care coordination, quality improvement, and regulatory standards.
  • Skilled in communication, conflict resolution, and team support.
  • Organized, resourceful, and proactive in managing multiple priorities.
  • Passionate about delivering compassionate, high-quality care to diverse populations, especially the elderly or frail.
