Clinical Coordinator RN Position Available In Onondaga, New York

Overview For a Limited Time Offering a $15,000 Sign-On Bonus Join Loretto Health and Rehabilitation as a Full Time Clinical Coordinator, RN! Under the guidance of the Unit Nurse Manager, The Clinical Coordinator is responsible for coordinating the clinical practice and care to residents in a Long Term Care and Short Term Rehab setting. This role is a liaison between residents, families, medical providers and Loretto employees. Conducts patient assessments, implements and evaluates the effectiveness of nursing care delivered to the residents.

Shift:

Full Time Days 7:30am

• 4:00pm or 8:00am
• 4:30pm

Pay:

$84,323.00

• $94,702.

00 Yearly Salary Responsibilities + Responsibilities include: + Oversee the delivery of Quality Patient Care + Compliance with NYSDOH and CMS regulations and facility policies and protocols + Orients, trains and mentors new employees + Provides health training to patients and their families + Assists in gathering and analyzing data for QAPI + Acts as primary liaison with medical providers Qualifications + 1+ year experience in nursing preferred + Current RN NYS license + Established patient assessment skills + Knowledge of best-practice standards + Skilled to respond to emergent situations + Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes + Exceptional interpersonal and customer service skills + Proficiency in speaking, listening, reading and writing the English Language is required Benefits/Compensation + Pay Range $84,323.00
