POSITION SUMMARY/PURPOSE

Care Management is acollaborative practice model including patients, nurses, socialworkers, physicians, healthcare team members,, caregivers and thecommunity. The Case Management process encompasses communicationand facilitates care along a continuum through effective resourcecoordination. The goals of the Care Management Team include theachievement of optimal health, access to care and appropriateutilization of resources balanced with the patient’s right toself-determination. Incumbents of this position are professionalRegistered Nurses that utilize the nursing process (assessment,planning, intervention, documentation, and evaluation) to determineand facilitate the most appropriate level of care andidentification of discharge needs. Collaborates with physician toascertain medical treatment plan and with nursing, other healthcare team members and health plans to fulfill the treatment plan inthe highest quality, cost effective manner. Responsible forensuring customer satisfaction (patient, family, physician,others), maintaining patient confidentiality and being sensitive tothe age and cultural specific patient needs for comfort, privacyand generalized care. .

ESSENTIAL FUNCTIONS

Incumbents of thisposition will strive to gain the respect and maintain the dignityof patients / family members, visitors and all Charlotte HungerfordHospital Personnel. Demonstrates a diplomatic and supportiveattitude and presents the hospital in a positive manner to allpersons above. Promotes and contributes, in a positive manner, tointer- and intra-departmental relationships to ensure all needs ofthe patient are met. Maintains confidentiality of all appropriateinformation and documentation. Responsible for complete andthorough patient and family assessments for initial and on-goingcare and discharge planning, addressing physical, psychosocial,religious, cultural and educational aspects. Identifies andcommunicates appropriate information gathered from patient, family,chart, community agencies and colleagues to other members of themultidisciplinary health care team. Actively coordinates patientcare including the sequencing and scheduling of tests, procedures,and consultations. Works closely with physicians to coordinatehospital services from pre-hospital through post dischargerecovery. Ensures patient and family understand the diagnosis andplanned course of treatment and determines and communicatesdischarge needs. Identifies gaps / barriers to care and facilitatesinterdepartmental communications to expedite appropriate changes.

Additionally, incumbents are responsible for assessing anyfinancial needs of the patient / family, including assisting in theclarification of benefit plans. Assists Social Worker Care Managerswith highly clinical complex patient needs. Responds expeditiouslyto emergency needs of patient and families in crisis by providingappropriate interventions necessary to support and stabilize. Inconjunction with patient, family, payers and other members of thehealth care team: formulates and implements a discharge plan toaddress assessed needs and patient/family concerns,; evaluates theeffectiveness of the plan in meeting the established care goals;and revises the plan as needed to achieve desired outcomes. Referspatients to a variety of resources including, but not limited to,Visiting Nurse Agency, Skilled Nursing facility, Long Term CareHospital, Rehab Facility, Hospice Care, Durable Medical andRespiratory Care providers. Assures Follow-up care with Providershas been arranged with confirmation of the patient’s ability toaccess the scheduled appointment. Telephone Contact to patient’swho are at a high risk for readmission within 48 hours of dischargeto verify the follow-up services has occurred as planned; Patientand Family understand and are following their dischargeinstructions and Patient and family are able to follow-up withtheir provider for evaluation. Pro-actively advocates for patientcare issues to ensure that overall quality and type of care issensitive to each specific patient/family’s needs. Analyzes patientcare trends and actively seeks out and collaborates with the careteam to improve overall quality and efficiency of care Seeks outand/or provides peer consultation about cases that are presentingproblems and or experiencing significant deviations from the planof care. Attends and participates in extended stay rounds, dailyoutcome planning rounds, Case Management Team meetings and peerreviews regarding management of their caseload. Participates inquality improvement and evaluation processes related to the CareManagement function. Abstracts data from the medical record toenable the review of selected hospital core measures or for reviewby external agencies such as the JCAHO, CMS and/or QIO. Collectsand trends a variety of data to identify avoidable hospital daysand/or process improvement opportunities. Meet required jobspecific competencies for assigned unit, including mandatoryeducational requirements. Maintains professional growth anddevelopment through seminars, workshops and professionalaffiliations to stay current on the latest trends related toposition. Participates in staff meetings, educational programs andin-service meetings to maintain competencies. Complies withhospital policies and procedures related to safety, infectioncontrol, attendance, sexual harassment, dress code, corporatecompliance, confidentiality and others. Communicates toAdministrative Director and/or Management staff needs identifiedfor self, the unit and other team members.

JOB KNOWLEDGE, SKILLREQUIREMENT AND REQUIRED ATTRIBUTES

Strong Communication andInterpersonal Skills Knowledge of the Nursing Process Ability tomanage multiple priorities and projects form initiation tocompletion within prescribed schedules and utilization ofresources. Ability to effectively utilize a variety of MicrosoftOffice, Outlook and Meditech HCIS software, Morrisey and Web basedprograms required. Skilled in Motivational Interviewing Ability andwillingness to follow the mission and values of the CharlotteHungerford Hospital on a daily basis Qualifications EDUCATIONMaintains current license as a Registered Nurse in the State ofConnecticut. Bachelor’s degree in Nursing preferred. EXPERIENCEMinimum of 3 years acute care hospital experience required. CareManagement experience preferred.
