Job Summary:

Coordinates a patient-focused team in providingdirect and collaborative health care in a variety of clinicalsettings designed to promote and restore optimal health across thelife span. Utilizes all components of the nursing process inproviding care that reflects an understanding of individual,cultural, and religious diversities and needs, and recognition ofpatient rights. Utilizes knowledge and expertise in case managementto provide comprehensive, coordinated, seamless care to individualsand aggregate groups. Coordinates, integrates, and facilitates thepatient/provider relationship along the continuum of care.

Responsibilities:

Able to identify immediate and long range plansfor patient care and includes both the patient and family in theplan Sets priorities for the patients needs, making decisions whichreflect sound judgement Monitor the patients clinical status andassess for significant change Communicate clearly with patient,family members, physicians, and team members to ensure patientneeds are met Safely administer medication on time and inaccordance with the Hospital’s policies Perform venipuncture andstarts intravenous lines safely Job Requirement Active RN licenseBachelor’s Degree – Nursing preferred BLS, PRI, certifications 4-5years Nursing experience Care Management UM experience Milliman andInterqual criteria Knowledge of Evidence-Based Criteria, DischargePlanning, Care Coordination, Community Resources
