Seeking an RN Care Manager to work needed at a medical center inQueens, Monday-Friday 8a-4p including weekends and holidays.

Qualified applicants must have a valid NYS RN license, vastunderstanding of insurance and payment systems, four (4) years ofacute Medical Surgical, Psychiatric, ED nursing and prior casemanagement experience.

Summary:

The RN Care Manager assesses,develops, implements, coordinates and monitors a comprehensive planof care for each patient/family in collaboration with thephysician, social worker, nurse and all members of theinterdisciplinary team for the inpatient, Emergency Department andall patient care areas. This position requires the ability tocombine clinical/quality considerations withregulatory/financial/utilization review demands to assure patientsare receiving care in the appropriate setting and level of care.

The Care Manager must understand insurance and payment systemregulations and requirements as they serve as a resource to thirdparty providers. Requirements Minimum of 4 years recent acuteMedical Surgical, Psychiatric, ED nursing or prior case managementexperience. Current N.Y.S. licensed Registered Nurse with BS, BSNor equivalent. ACM or

CCM REQUIRED.

Patient Review Instrument (PRI)certification is

REQUIRED.

Knowledge of payer eligibility andreimbursement regulations and the impact on the continuum of care.

Knowledge of screening criteria i.e. Inter

QUAL, MCG

criteria.

Knowledge of discharge planning regulations and community resourcesto coordinate safe discharges from the inpatient units and the EDto outpatient settings. Excellent negotiation andinterpersonal/communication skills and the ability to workeffectively with others on the team to achieve desired outcomes.

Organizational time management skills as evidenced by capacity tomultitask and prioritize work. Ability to work in a collaborativepartnership model with Social Workers and other members of theinterdisciplinary team both internal and external.
