CONTRACT NOTE:

Shift is Monday

  • Friday 7am-3pm or 8am-4pm.
MUST HAVE EXPERIENCE PAYRATE

LOCAL/PER DIEM

  • $40/HR TRAVEL
  • $29/HR
TRAVEL NO HOUSING/SPLIT PAY
  • $20/hr (taxable) + $960(tax-free) $1720/40hours (20/23-43)
Job Description:

Coordinate and manage the completion of the Resident Assessment Instrument (RAI) and Minimum Data Set (MDS) per federal and state guidelines. Ensure timely and accurate submission of MDS assessments in accordance with CMS regulations. Oversee care planning process, including initial, quarterly, annual, and significant change assessments. Collaborate with interdisciplinary team members to develop individualized care plans for residents. Monitor documentation to support MDS coding and reimbursement levels under PDPM. Serve as liaison during audits and surveys related to MDS or clinical documentation. Educate nursing and ancillary staff on documentation requirements and regulatory updates. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives. Maintain confidentiality and protect resident rights in accordance with HIPAA and facility policy.

Qualifications:

Valid Registered Nurse (RN) license in Pennsylvania, in good standing. Minimum of 1-2 years

MDS/RNAC

experience in a long-term care or skilled nursing setting preferred. Strong understanding of

PDPM, OBRA

regulations, and CMS guidelines . Experience with electronic health record (EHR) systems , such as PointClickCare, MatrixCare, or similar. Excellent attention to detail, organizational, and time management skills. Strong written and verbal communication skills; able to work effectively in a team environment. Certification as a Resident Assessment Coordinator (RAC-CT) through AANAC is a plus but not required. Requirements Resume Current
