JOB DESCRIPTION
Case Manager will work in remote and fieldsetting supporting our Medicaid SMI (Severe Mental Illness)Population in the state of South Carolina. Case Manager will berequired to communicate telephonically and complete Face to Facemeetings. You will participate in interdisciplinary care teammeetings for our members and ensure they have care plans based ontheir concerns/health needs. Excellent computer skills andattention to detail are very important to multitask betweensystems, talk with members on the phone, and enter accurate contactnotes. This is a fast-paced position and productivity is important.
TRAVEL (up to 60%) in the field to do member visits in thesurrounding areas will be required. (Looking for candidates NorthSouth Carolina) Home office with internet connectivity of highspeed required.

Monday thru Friday 8:00AM to 5:00PM EST
- No weekends or Holiday Job Summary Molina Healthcare Services(HCS) works with members, providers and multidisciplinary teammembers to assess, facilitate, plan and coordinate an integrateddelivery of care across the continuum, including behavioral healthand long term care, for members with high need potential.
HCS staffwork to ensure that patients progress toward desired outcomes withquality care that is medically appropriate and cost-effective basedon the severity of illness and the site of service.
KNOWLEDGE/SKILLS/ABILITIES
- Completes comprehensive assessments ofmembers per regulated timelines and determines who may qualify forcase management based on clinical judgment, changes in member’shealth or psychosocial wellness, and triggers identified in theassessment.
- Develops and implements a case management plan incollaboration with the member, caregiver, physician and/or otherappropriate healthcare professionals and member’s support networkto address the member needs and goals.
- Conducts face-to-face orhome visits as required.
- Performs ongoing monitoring of the careplan to evaluate effectiveness, document interventions and goalachievement, and suggest changes accordingly.
- Maintains ongoingmember case load for regular outreach and management.
- Promotesintegration of services for members including behavioral healthcare and long term services and supports/home and community toenhance the continuity of care for Molina members.
- Facilitatesinterdisciplinary care team meetings and informal ICTcollaboration.
- Uses motivational interviewing and Molina clinicalguideposts to educate, support and motivate change during membercontacts.
- Assesses for barriers to care, provides carecoordination and assistance to member to address concerns.
- 25-40% local travel required.
- RNs provide consultation,recommendations and education as appropriate to non-RN casemanagers.
- RNs are assigned cases with members who have complexmedical conditions and medication regimens
- RNs conduct medicationreconciliation when needed.
JOB QUALIFICATIONS
Required EducationGraduate from an Accredited School of Nursing. Bachelor’s Degree inNursing preferred. Required Experience 1-3 years in casemanagement, disease management, managed care or medical orbehavioral health settings. Required License, Certification,Association Active, unrestricted State Registered Nursing (RN)license in good standing. Must have valid driver’s license withgood driving record and be able to drive within applicable state orlocality with reliable transportation. Preferred EducationBachelor’s Degree in Nursing Preferred Experience 3-5 years in casemanagement, disease management, managed care or medical orbehavioral health settings.
