We are looking for an Infertility/Maternity Nurse Care Managerfor our Care and Case Management team who also holds a CCM and a CAlicense. You must have case and disease management in a healthplan, health navigator or third-party administrator (TPA)environment. You must be passionate about holistic,patient-centered care to support members through their healthcarejourney and ensure needs are met with industry-leadinginterventions. As a telephonic Nurse Care Manager you willspecialize in guiding members through both routine and complexinfertility and maternity clinical scenarios. You will report tothe Manager, Care and Case Management and partner with a diverseclinical team, including providers, care coordinators, and othersupporting team members, to deliver integrated remote care and casemanagement. You should enjoy spending time on the phone, listeningto members needs, answering questions, and serving as a memberadvocate. You will excel at creating personalized care plans andhave the clinical skills to guide members through infertility andmaternity journeys, while navigating available benefits andresources. As a CCM-certified nurse, you will be equipped with theexpertise to navigate complex healthcare systems and ensure thateach member receives the highest quality care. Nurse Care Managerswill support members with education, advocacy, and care managementthrough family-building/fertility journeys and prenatal/postpartumcare, ensuring they receive comprehensive care that results inpositive health outcomes for both them and their families.
Responsibilities Deliver coordinated, patient-centered virtual CareManagement by telephone or messaging that improves members healthoutcomes. Create impactful care plans together with members and ourdiverse care team, and help members achieve their desired goals.
Help members navigate complex medical conditions, treatmentpathways, benefits, and the healthcare system in general. Partnerwith the members local providers to ensure coordinated care.
Provide compassionate, longitudinal follow-up care, buildingsupportive relationships.
Qualifications and Requirements:
Bachelorof Science in Nursing (BSN). Must reside in a compact NLC state.
Active Compact RN license in good standing with the nursing boardof their state. Active California Nursing License requiredWillingness to become licensed in multiple states. Must havecurrent CCM Certification or eligibility to sit exam in August 20253 years of experience in nursing 2 years experience working incare, case and disease management 2 years experience working inlabor and delivery 2 years experience working in infertility Becomfortable discussing different medical conditions Spanishspeaking desirable Experience with technology and an understandingof digital tools and EMR platforms Strong empathy and commitment topatient-centered care. Strong competence and abilityto use multiple computer/medical record systems. Physical/Cognitive Requirements Prompt and regular attendance atassigned work location. Capability to remain seated in a stationaryposition for prolonged periods. Eye-hand coordination and manualdexterity to operate keyboard, computer and other office-relatedequipment. No heavy lifting is expected, though occasional exertionof about 20 lbs of force (e.g., lifting a computer / laptop) may berequired. Capability to work with leadership, employees, andmembers in an appropriate manner. The United States new hire basesalary target range for this full-time position is: $73,700 -$103,180 equity benefits This range reflects the minimum andmaximum target for new hire salaries for candidates based on theirrespective Zone.
