The Nurse Care Manager Trainer/Floater is part of theperson-centered integrated care team and provides direct clinicaland medical services to persons enrolled in the Health Care Homethrough a “whole person” approach. This position assists inproviding training/onboarding to new Nurse Care Managers (NCM) andfills in for NCMs where needed (virtually and/or face-to-face).This is a full-time, benefit eligible position that will workMonday-Friday, primarily 8:00 am – 5:00 pm. It is remote, but matrequire minimal travel.
ESSENTIAL FUNCTIONS – JOB SPECIFIC
- Withthe Sr. Director of Nursing Services, develop the onboardingschedule for new DD HH NCMs (NCMs)
- Provide onboarding to new NCMsin collaboration with their Regional Nurse Director (RND) to ensureconsistency in training throughout the program
- Provide refreshertrainings as needed
- Regularly seek feedback on the onboardingprocess and make changes to ensure efficiency and effectiveness ofthe process
- With the Sr. Director of Nursing Services, developand maintain DD HH nursing processes
- As part of the onboardingand resignation process, obtain, maintain, and delete access forthe following: Cyberaccess, HRST, Care Manager, Lab Portals, andContact Profiles
- Serve as the point person for New EmployeeOrientation along with the RND
- Provide NCM services whereneeded/as needed (i.e. for NCMs on leave, PTO, where extra help isneeded, etc.)
- Responsible for the completion of monthly assignedinterventions, and all aspects of care according to Department ofMental Health regulations for the DD Health Home Program for acaseload of up to 200 clients
- Meet with assigned clients tocomplete health screening initially and annually in an office-basedor community-based setting; identify risk level, care gaps and makeclinical care recommendations
- Assist in developing a physicalhealth goal for the service plan; promote healthy lifestyleincluding healthy nutrition, physical activity, and freedom fromtobacco use
- Ensure that clients are referred to a PCP orspecialist, if they do not have one; provide education toclients/caregivers regarding the importance of having a PCP
- Complete metabolic syndrome screenings on assigned clientsfollowing the metabolic screening policy and standing order
- Perform medication reconciliation for assigned clients who havebeen discharged from the hospital per agency guidelines
- Provideeducation to clients and caregivers on monthly reports, and asneeded, regarding physical health conditions, treatment, screening,prevention, and self-management
- Provide consultation andeducation for other care team members regarding chronic healthconditions
- Consult with Specialized Healthcare Consultant andPrimary Care Physician Consultant as scheduled and as needed
- Assist in contacting/collaborating with medical providers and otherhuman service agencies regarding significant health information toensure the provision of holistic services for assigned clients
- Travel to client’s place of residence as needed
- Meet serviceprovision expectations per client needs and company guidelines
- Will seek culturally competent services and service providers tomeet the unique cultural needs of each client
- Travel to clienthomes/community locations for nursing coverage and training asneeded
- Other duties as assigned
