The Occupational Therapist in HomeHealth is responsible for the assessment and evaluation of patientcare needs related to functional status, activities of dailyliving, fine motor coordination, home assessments and adaptiveequipment, and other occupational therapy needs as defined bymedical conditions. Provides service within the scope of practice as defined by thestate laws governing the practice of Occupational Therapy, inaccordance with the plan of care, using evidence-based techniques,and in coordination with other members of the health careteam. Evaluates the patient’s functional status, status of all bodysystems as required for CMS documentation, and occupational therapyneeds. Consults with the physician in the development of thetherapy plan of care. Observes, records, and reports to the nurse supervisor and thephysician the patient’s response to treatment and changes to thepatient’s condition. Conducts patient assessments evaluating the level of functionby applying diagnostic and prognostic functional ability tests. License Requirements Current Occupational Therapy licensure in state of Current CPR certification Current driver’s license, vehicle insurance, and access to adependable vehicle, or public transportation
