Position Overview We are looking for an Occupational Therapist(OT) to join our team! You will utilize your clinical expertise toassess, teach, and deliver therapy interventions and treatmentdirected by the Physician as established in the home health plan ofcare. You will collaborate with interdisciplinary team members,receiving and providing pertinent updates to optimize patientwell-being.

Schedule:

Full-Time visits Monday –

Friday Territory :

Limestone, Lauderdale, Morgan, and Lawrence counties

Essential JobFunctions:

Perform initial and ongoing assessments to evaluatemobility, coordination, strength and current level of function.

Responsible for administration, evaluation, and interpretation oftests and measurements to determine functional goals and outcomes.

Provide a written evaluation and plan of care defining occupationaltherapy needs, goals, progress and expected outcomes. Delivery ofoccupational therapy services to comply with prescribed physicianorders. Maintains plan of care by performing timely documentationof client performance, modifying goals and treatment as needed, anddetermine discharge care plans as appropriate. Responsible fordischarge planning, including collaborating and educating thepatient and his/her caretakers on home exercise plan to followdischarge. Participates in coordination of services with healthcare team, physicians, parent/guardians, nurses, etc. Documentsaccording to policy and procedure and requires minimal supervisionor instruction. Documents effectively resulting in no technical orclinical denials subsequent to review of documentation by payersMaintain effective communication between staff, healthcare teammembers and family. Serve as a resource consultant for clinicalstaff. Supervises other personnel (OTA, HHA) as applicable.

Completes OASIS assessments where allowed by state professionalpractice and regulation.

Requirements:

Degree in OccupationalTherapy from an accredited university/program. Current,unrestricted state license as an Occupational Therapist in thestate of practice

Valid CPR Preferred:
