Position Type PRN Empath Home Health, a member of Empath Health, is seeking a PRN Home Health OT (Occupational Therapist). The Home Health OT•Occupational Therapist provides occupational therapy services related to patient’s level of function and performance of daily living.
Locations:
home health visits available in Collier county (Naples, Marco Island, Immokalee, and/or Ave Maria)
Schedule:
PRN The Home Health OT•
Occupational Therapist:
Provides treatment to the patient in compliance with agency policies and procedures and federal and state regulations.
Performs initial and on going assessments to determine level of functioning and patient’s ability to perform activities of daily living, including OASIS assessments at appropriate time points.
Develops and revises a plan of care in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of self-care.
Requirements:
Bachelor Degree in Occupational Therapy.
Florida licensure in occupational therapy.
Minimum of one (1) year experience as a licensed Occupational Therapist.
