ResponsibilitiesResponsible for all aspects ofoccupational therapy service delivery provided under a physicianapproved plan of care.

Ensures the plan of care addresses allproblems identified in the assessment or documents rationale ifidentified problems are not addressed in the plan ofcare.

Accountable for the safety and effectiveness of theoccupational therapy service delivery process.

Completesdocumentation timely and accurately.

Supervises home health aidewhen RN is not active in the case.

Other duties asassigned.

QualificationsCurrent license to practice occupationaltherapy specific to the state you are assigned to work.

Graduate ofan occupational therapy program accredited by the AccreditationCouncil for Occupational Therapy Education (ACOTE) of the AmericanOccupational Therapy Association, Inc. (AOTA), or successororganizations of ACOTEEligible to take, or has successfullycompleted, the entry-level certification examination foroccupational therapists developed and administered by the NationalBoard for Certification in Occupational Therapy, Inc. (NBCOT).Oneyear related occupational therapy experience to ensure knowledgeand skills are sufficient to safely provide occupational therapyservices to patients.

If less than one (1) year experience, thenapproval from regional clinical leaders required as well asappropriate mentoring and/or residency program established.

CurrentCPR certification.
