The Occupational Therapist provides high-quality,patient-centered care by managing the complete process ofoccupational therapeutic care of an assigned group of patients, incoordination with the physician and the patient care team. TheOccupational Therapist completes patient assessments, andparticipates in developing and implementing individualized careplans, focusing on goal achievement, patient engagement, andpositive patient experience. The Occupational Therapist provides ordirects a majority of the patient’s physical therapy to supportpatients throughout home health and the continuum of care.
What youwill do in this role: You will provide evaluations and treatment interventions withinthe scope of practice established by law and professional code ofethics. You will assess and evaluatetherapeutic/rehabilitative/functional status. You will participatein patient placement and in the development of the total plan ofcare. You will provide purposeful and skilled interaction care. Youwill use procedures and techniques to produce a change incondition/function consistent with the physician’s plan ofcare. You will observe and report patient’s reaction to treatment andchanges in patient’s condition. You will communicate pertinent payor needs/requirements tophysician, treatment team, patient and patient’s family in a timelymanner. You will prioritize caseload, determine daily staffing needsand coordinate assignments/handoffs.
