Home Health Occupational Therapist 3.7 3.7 out of 5 stars Ocala, FL Overview We are seeking a dedicated and compassionate Occupational Therapist to join our team. In this role, you will work with patients of all ages to enhance their ability to perform daily activities and improve their overall quality of life. You will be responsible for developing individualized care plans, implementing therapeutic interventions, and collaborating with other healthcare professionals to ensure comprehensive patient care. This position offers the opportunity to work in various settings, including assisted livings, Independent Livings, and patient homes. Responsibilities Assess patients’ needs through comprehensive evaluations and observations. Develop and implement personalized care plans tailored to each patient’s goals. Provide therapeutic interventions that may include activities of daily living (ADLs), fine motor skills training, and sensory integration techniques. Collaborate with interdisciplinary teams, including speech therapists and case managers, to coordinate care. Educate patients and their families on strategies to enhance independence and safety at home or in school environments. Document patient progress and modify treatment plans as necessary based on ongoing assessments. Assists the physician in evaluating level of function, helps develop the plan of care(revising as necessary), advises and consults with the family and other agency personnel. Provides occupational therapy services as prescribed by the physician, which can be safely provided in the home and to assist the physician in evaluating the patient’s level of function by applying diagnostic and therapeutic procedures. Guides the patient in the use of therapeutic, creative, and self-care activities for the purpose of improving function. Maintain compliance with occupational health standards and clinic policies. Qualifications A Master’s or Doctorate degree in Occupational Therapy from an accredited program in the state of Florida. Valid state licensure or certification as an Occupational Therapist. Knowledge of speech therapy practices is beneficial for holistic patient care. Strong communication skills to effectively interact with patients, families, and healthcare team members. Ability to develop and implement effective care plans based on individual patient needs. Familiarity with case management principles and early intervention strategies is advantageous. Commitment to ongoing professional development and staying current with industry best practices.

$65.00 – $95.00 per hour
