Location:

We have needfor OTs to perform home visits in one or more of the following teamareas:•

Team Orange:

West Orlando, and Ocoee, Winter Garden,Windermere area The role you’ll contribute: The Home HealthOccupational Therapist (OT), under the general supervision of theCare Manager, provides occupational therapy to assigned patientsand directs therapy assistants and Home Health Aides to providequality care in accordance with physician orders under a plan ofcare. Assesses, plans, implements, evaluates total patient care andcoordinates patient/family teaching. Assures compliance withtherapy standards, federal, state, and local governmentregulations, and company clinical policies. Provides goal directedcare leading to specific outcomes. May be on call for after-hourswork. Coordinates and directs the home care patient’s care based onindividual patient needs. The value you’ll bring to the team:

  • Performs an evaluation, assessing function using a method whichobjectively measures activities of daily living such as, but notlimited to, eating, swallowing, bathing, dressing, toileting,walking, climbing stairs, using assistive devices and mental andcognitive factors, documenting the measurement results in theclinical record.
  • Administers treatments as discussed in the planof care involving application of physical agents, and manualtechniques to improve and maintain function. Instructs, motivates,and assists the patient to perform various functional physicalactivities, activities in daily living, self
  • care activities, andexercises.
  • Measures and monitors effects of treatment at variousstages and confers with occupational therapist to adjust treatmentsto achieve maximum benefit and outcome.
  • Assures that patienttreatment outcome is directed towards development and restorationof functional skills and returning to or maintaining maximumpotential.
  • Recommends and teaches the use of assistive devicesand equipment to patients and families.
  • Instructs nurses, HomeHealth Aides, and family members to provide for continuousOccupational Therapy program.
  • Observes records and reports onpatient progress, reaction to treatment and any changes inpatient’s condition. The expertise and experiences you’ll need tosucceed: Minimum qualifications:
  • Minimum of six-months relevantoccupational therapy
  • Occupational Therapist (OT)
  • ActiveAmerican Heart Association BLS
  • Valid in state Driver’s Licensewith current car insurance Preferred qualifications:
  • Recent,relevant experience in a Medicare-certified home health agency as aOccupational Therapist

The salary range reflects the anticipated base payrange for this position. Individual compensation is determinedbased on skills, experience and other relevant factors within thispay range. The minimums and maximums for each position may varybased on geographical location.
