Full Time Shift :

Monday-Friday, 8am – 5pm, flexible schedule

Location:

We have needfor OTs to perform home visits in one or more of the following teamareas:

Team Blue:

W. Longwood, Lake Mary, Apopka, Mt Dora area Therole you’ll contribute: The Home Health Occupational Therapist(OT), under the general supervision of the Care Manager, providesoccupational therapy to assigned patients and directs therapyassistants and Home Health Aides to provide quality care inaccordance with physician orders under a plan of care. Assesses,plans, implements, evaluates total patient care and coordinatespatient/family teaching. Assures compliance with therapy standards,federal, state, and local government regulations, and companyclinical policies. Provides goal directed care leading to specificoutcomes. May be on call for after-hours work. Coordinates anddirects the home care patient’s care based on individual patientneeds. The value you’ll bring to the team: Performs an evaluation,assessing function using a method which objectively measuresactivities of daily living such as, but not limited to, eating,swallowing, bathing, dressing, toileting, walking, climbing stairs,using assistive devices and mental and cognitive factors,documenting the measurement results in the clinical record.

Administers treatments as discussed in the plan of care involvingapplication of physical agents, and manual techniques to improveand maintain function. Instructs, motivates, and assists thepatient to perform various functional physical activities,activities in daily living, self- care activities, and exercises.

Measures and monitors effects of treatment at various stages andconfers with occupational therapist to adjust treatments to achievemaximum benefit and outcome. Assures that patient treatment outcomeis directed towards development and restoration of functionalskills and returning to or maintaining maximum potential.

Recommends and teaches the use of assistive devices and equipmentto patients and families. Instructs nurses, Home Health Aides, andfamily members to provide for continuous Occupational Therapyprogram. Observes records and reports on patient progress, reactionto treatment and any changes in patient’s condition. The expertiseand experiences you’ll need to succeed: Minimum qualifications:

Minimum of six-months relevant occupational therapy OccupationalTherapist (OT) Active American Heart Association BLS Valid in stateDriver’s License with current car insurance

Preferredqualifications:

Recent, relevant experience in a Medicare-certifiedhome health agency as a Occupational Therapist Master’s inOccupational Therapy This facility is an equal opportunity employerand complies with federal, state and local anti-discriminationlaws, regulations and ordinances.
