The position major duties and responsibilities are: Based on the findings from a thorough assessment, plans an effective goal-directed trement program th is approprie and indiced for the disability/condition being treed. Depending on the setting, proficient in the use of VA Video conferencing (VVC) technology and use of Home Exercise Software to educe the pient. Crees a plan of care and refers trement to an Occupional Therapist Assistant when approprie. Works closely with an interdisciplinary team in the planning and implementing of the pient’s trement program. When assessing or providing client care, the therapist considers the age of the client as it reles to the ability to understand directions, provide objective self-assessments, or any other medical milieu approprie to the overall trement plan. Provides clients and families with counseling, educion, informion, and training regarding the nure of their diseases or disability, trement plans and goals, home program, and post program adjustment. This is accomplished individually or with the interdisciplinary team, family and client. Recognizes contraindicions and approprieness of trement prescribed by the primary provider and consults with provider if these are in question. Appropriely plans client discharge and/or need for continuing trement and termines trement when the client reaches maximum benefit.
