Location:

We have needfor OTs to perform home visits in one or more of the following teamareas:•

Team Pink:

East Orlando, South Sanford, Winter Springs,Oviedo, East Longwood, Waterford, Christmas area The role you’llcontribute: The Home Health Occupational Therapist (OT), under thegeneral supervision of the Care Manager, provides occupationaltherapy to assigned patients and directs therapy assistants andHome Health Aides to provide quality care in accordance withphysician orders under a plan of care. Assesses, plans, implements,evaluates total patient care and coordinates patient/familyteaching. Assures compliance with therapy standards, federal,state, and local government regulations, and company clinicalpolicies. Provides goal directed care leading to specific outcomes.

May be on call for after-hours work. Coordinates and directs thehome care patient’s care based on individual patient needs. Thevalue you’ll bring to the team:

  • Performs an evaluation, assessingfunction using a method which objectively measures activities ofdaily living such as, but not limited to, eating, swallowing,bathing, dressing, toileting, walking, climbing stairs, usingassistive devices and mental and cognitive factors, documenting themeasurement results in the clinical record.
  • Administerstreatments as discussed in the plan of care involving applicationof physical agents, and manual techniques to improve and maintainfunction. Instructs, motivates, and assists the patient to performvarious functional physical activities, activities in daily living,self
  • care activities, and exercises.
  • Measures and monitorseffects of treatment at various stages and confers withoccupational therapist to adjust treatments to achieve maximumbenefit and outcome.
  • Assures that patient treatment outcome isdirected towards development and restoration of functional skillsand returning to or maintaining maximum potential.
  • Recommends andteaches the use of assistive devices and equipment to patients andfamilies.
  • Instructs nurses, Home Health Aides, and family membersto provide for continuous Occupational Therapy program.
  • Observesrecords and reports on patient progress, reaction to treatment andany changes in patient’s condition. The expertise and experiencesyou’ll need to succeed: Minimum qualifications:
  • Minimum ofsix-months relevant occupational therapy
  • Occupational Therapist(OT)
  • Active American Heart Association BLS
  • Valid in stateDriver’s License with current car insurance
Preferredqualifications:
  • Recent, relevant experience in aMedicare-certified home health agency as a Occupational Therapist
