Gwinnett County, GA Job Summary:

We are seeking a skilled and compassionate Occupational Therapist (OT) to join our home health team. The OT will be responsible for providing comprehensive occupational therapy services to patients in their homes to help them regain independence and improve their quality of life. The ideal candidate will have a strong background in occupational therapy, excellent clinical skills, and a passion for delivering personalized care to patients in a home setting.

Responsibilities:

1. Conduct comprehensive evaluations of patients’ functional abilities, including assessments of activities of daily living (ADLs), mobility, cognition, and psychosocial factors. 2. Develop individualized occupational therapy treatment plans based on patient assessments, goals, and physician orders, with input from the patient and their caregivers. 3. Provide skilled occupational therapy interventions to improve patients’ strength, coordination, balance, range of motion, and fine motor skills, using evidence-based techniques and modalities. 4. Design and implement therapeutic exercises, activities, and adaptive equipment recommendations to enhance patients’ independence and functional capacity in their home environment. 5. Educate patients and their caregivers on safe and effective techniques for performing ADLs, using assistive devices, and adapting their home environment to maximize independence and safety. 6. Monitor patients’ progress and responses to therapy interventions, adjusting treatment plans as needed to achieve optimal outcomes and meet established goals. 7. Document all occupational therapy evaluations, interventions, progress notes, and discharge summaries accurately and in a timely manner in electronic medical records. 8. Collaborate with interdisciplinary healthcare team members, including physicians, nurses, physical therapists, and social workers, to coordinate patient care and achieve holistic treatment goals. 9. Provide emotional support, encouragement, and counseling to patients and their families, addressing their concerns and promoting psychosocial well-being. 10. Ensure compliance with regulatory requirements, agency policies, and standards of occupational therapy practice, including infection control protocols and safety regulations. 11. Participate in ongoing professional development activities, including continuing education courses, workshops, and in-service training programs. 12. Maintain professionalism, confidentiality, and ethical standards in all interactions with patients, families, and colleagues.

Qualifications:

1. Current license to practice as an Occupational Therapist (OT) in the state of Georgia. 2. Minimum of one year of clinical experience as an Occupational Therapist, preferably in home health or a related setting. 3. CPR certification required 4. Strong clinical assessment, treatment planning, and documentation skills. 5. Excellent communication and interpersonal skills, with the ability to establish rapport with patients and their caregivers. 6. Proficiency in using electronic medical records systems and other healthcare technology. 7. Valid driver’s license and reliable transportation required. 8. Ability to work independently, prioritize tasks, and manage time effectively in a dynamic home care environment. 9.
