Job Responsibilities andRequirements:
PRIMARY RESPONSIBILITIES
Assesses patient ‘s ability to safely and effectively performindependent living skills. Designs the appropriate plan of care,including type and frequency of treatment. Implements the treatment plan using appropriate modalities.
Documents findings of patient progress. Monitors and modifies theplan of care as needed. Seeks consultation as necessary. Educates the patient and family/caregiver about patientdeficits. Plans and/or assists with patient discharge from therapyservices and provides information regarding appropriateselection/use of adaptive equipment and support programs. Oversees the care provided by designated individuals accordingto established regulations and practice standards. Works in a constant state of alertness and safe manner. Performs other duties as assigned.
EDUCATION
Bachelor’s, Master’s or Doctorate degree in OccupationalTherapy, or related area as required to obtain licensure in stateof practice

PHYSICAL REQUIREMENTS
Constant standing. Frequent lifting/carrying and pushing/pulling objects weighing0-25 lbs. Frequent use of hearing and speech to share information throughoral communication. Frequent use of hearing to distinguish body sounds and/or hearalarms, malfunctioning machinery, etc. Frequent use of vision for distances near (20 inches or less)and far (20 feet or more). Frequent walking, reaching, gripping and keyboard use/dataentry. Occasional bending, stooping, climbing, kneeling, sitting,squatting, twisting and repetitive foot/leg and hand/armmovements. Occasional lifting/carrying and pushing/pulling objectsweighing 25-50 lbs. Occasional lifting/moving patients. Occasional use of vision to judge distances and spatialrelationships and to identify and distinguish colors. Occasional driving. Rare crawling.
REQUIRED PROFESSIONAL LICENSE
AND/ORCERTIFICATIONSResuscitation Certification Requirement:
Basic LifeSupport Healthcare Provider (BLS HCP) required within 90 day ofemployment. Other resuscitation certifications may be requireddependent on department.
