Job Description Job Description BAYADA Home Health Care iscurrently seeking an experienced Occupational Therapist, OT, for apart time/ per diem opportunity performing home health visits forour Concord, located in Concord, New Hampshire. This officeservices adult and geriatric patients on a per visit basis in homesthroughout Plymouth and surrounding areas. As a home careOccupational Therapist, you will be an integral member of amulti-disciplinary health care team that provides rehabilitativecare and skilled nursing to clients, affording them the opportunityto receive the medical care required to remain at home.
Occupational Therapist, OT Responsibilities:
- Make home visits toclients in designated geographic territories.
- Perform thoroughevaluations of patients’ functional status, including musclefunction, endurance, visual coordination, written and verbalcommunication skills, and ADL and IADL performance.
- Assesscognitive, psychological and motor aspects of performance anddevelop individualized intervention plans to achieve favorableoutcomes.
- Evaluate home environment for hazards or barriers tomore independent living.
- Identify equipment needs andenvironmental adaptations resulting in enhanced outcomes forclients.
- Continually assess and revise the occupational therapycare plan, and participate with nursing in the multidisciplinarycare plan, as appropriate.
- Educate and instruct patients, familymembers, or other patient representatives, in rehabilitative careand activities necessary to promote the client’s health, safety andindependent living.
- Accurately document observations,interventions, and evaluations pertaining to patient caremanagement and services provided, utilizing a state-of-the-arttouch pad tablet.
Occupational Therapist, OT Qualifications:
- Acurrent New Hampshire Occupational Therapist license.
- Prior homecare experience preferred, but not required
- A minimum of one yearof recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointlyby the Committee on Allied Health Education and Accreditation ofthe AMA and the AOTA, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problemsolving skills.
- Solid computer skills; prior experience withelectronic medical records (EMR) preferred. BAYADA believes thatour employees are our greatest asset:
