Salaried position with a $15,000 Sign-On Bonus Position OverviewWe are looking for an Occupational Therapist (OT) to join our team!

You will utilize your clinical expertise to assess, teach, anddeliver therapy interventions and treatment directed by thePhysician as established in the home health plan of care. You willcollaborate with interdisciplinary team members, receiving andproviding pertinent updates to optimize patient well-being.

Essential Job Functions:

• Perform initial and ongoing assessmentsto evaluate mobility, coordination, strength and current level offunction.
• Responsible for administration, evaluation, andinterpretation of tests and measurements to determine functionalgoals and outcomes.
• Provide a written evaluation and plan of caredefining occupational therapy needs, goals, progress and expectedoutcomes.
• Delivery of occupational therapy services to complywith prescribed physician orders.
• Maintains plan of care byperforming timely documentation of client performance, modifyinggoals and treatment as needed, and determine discharge care plansas appropriate.
• Responsible for discharge planning, includingcollaborating and educating the patient and his/her caretakers onhome exercise plan to follow discharge.
• Participates incoordination of services with health care team, physicians,parent/guardians, nurses, etc.
• Documents according to policy andprocedure and requires minimal supervision or instruction.
• Documents effectively resulting in no technical or clinical denialssubsequent to review of documentation by payers
• Maintaineffective communication between staff, healthcare team members andfamily. Serve as a resource consultant for clinical staff.
• Supervises other personnel (OTA, HHA) as applicable.
• CompletesOASIS assessments where allowed by state professional practice andregulation.

• Degree in Occupational Therapy from anaccredited university/program.
• Current, unrestricted statelicense as an Occupational Therapist in the state of practice
