Position Summary:

The primary function of your job is toreturn patient to optimal physical function and facilitate theachievement of patients, physicians, and therapy goals within a SNFsetting. Instruct the patient in maintaining achieved level offunction after discharge through education, independent exerciseprogram, and appropriate activity modification.

Education/Qualifications:

• Employee must possess a Bachelor’sDegree or Master’s Degree in Occupational Therapy from anaccredited college or university.
• Must possess a current andvalid license granted from state of employment.
• Must possessbasic computer skills and the ability to navigate through varioussoftware programs, including service delivery software and theelectronic health record software.
