The Occupational Therapist II evaluates patient?s conditions,plans, organizes, and participates in rehabilitative programs thatfacilitates return to optimal functional abilities, improvemobility, relieve pain, increase strength, and decrease or preventdeformity of patients suffering from disease or injury, includingpatient and family education. This position is able to effectivelyand efficiently treat all patients in their clinical practicesetting and complete all occupational therapy job functions withminimal guidance. This therapist initiates communication amongstinter-disciplinary team members, treats familiar patients withconfidence, and advocates for their patients and team. Thistherapist may require mentoring/guidance in complex situations andmay have difficulty with cross coverage to other settings. Evaluates and treats all diagnoses and more complex patients. Conducts thorough evaluations of patients’ physical abilities,medical history, and functional limitations to developindividualized treatment plans. Administers therapeutic exercises and other occupational therapyinterventions to improve patients’ strength, flexibility, and rangeof motion. Educates patients and their families on treatment plans, homeexercises, and self-management techniques to improve recoveryoutcomes. Collaborates with other in developing and implementing dischargeplans and recommendations for continued care and may require someguidance from other team members. Monitors and documents patients’ progress, treatment outcomes, andresponses to interventions so that treatment plans can be adjustedas needed. Collaborates on teams from different departments to ensurecoordinated patient care and holistic treatment outcomes. Performs other duties as assigned.
