HOME CARE OCCUPATIONAL THERAPIST

(OT)

PartTime Job Description:

Provides patient care services along withdirect treatment and follow-up. Evaluates patients new to servicewhere there are physician orders, utilizing motor evaluation,sensory-perceptual-cognitive evaluation, and/or performanceevaluation as indicated by the patient’s condition. Develops a planof care appropriate to the patient’s diagnosis, utilizingassessment findings and data. Establish measureable goals incompliance with patient’s physical assessment. Documents patientcare data accurately, thoroughly, and in a timely matter,reflecting evidence of improvement/progress/decline in patient’scondition. Coordinate regularly with COTA’s and provide monthlysupervision of the COTA’s patient care. Submits the daily/weeklypatient visit schedule to the Scheduler. Communicates as necessarywith intercompany and intracompany contacts. Discusses company andthird party reimbursement policies and guidelines with thepatient/family and discuss treatment plans with the Patient CareCoordinator, physician, and patient/family. Coordinates withPatient Care Coordinator in making referrals to appropriatedisciplines. Provides in-service education, health careprofessional education, and public education in addition tosupervising peers and supporting personnel and students in trainingas needed by the company. Conducts research and assists withorientation of new employees as needed.

Job Requirements:

Minimumbachelor’s degree in occupational therapy from an accredited schoolof occupational therapy, successful completion of the NationalCertification Exam and current licensure from the state wheretherapy will be provided. Prefer a minimum of one year’stherapeutic experience in a non-home health care environment.

Strong knowledge of theory and concepts relating to the practice ofphysical therapy and the ability to evaluate and treat patients forwhom a physician’s order exists. CPR certified and strongcommunication and organization skills. Minimum of twelve hours ofcontinuing education annually. Accept travel and working conditionsas assigned.
