opportunity performing home health visits forour Person County Visits office, located in Roxboro, NorthCarolina. This office services adult and geriatric patients on aper visit basis in homes throughout Granville, Vance, Franklin,Person, Caswell, and Orange counties. As a home care OccupationalTherapist, you will be an integral member of a multi-disciplinaryhealth care team that provides rehabilitative care and skillednursing to clients, affording them the opportunity to receive themedical care required to remain at home.

Occupational Therapist, OTResponsibilities:

Make home visits to clients in designated geographicterritories. Perform thorough evaluations of patients’ functional status,including muscle function, endurance, visual coordination, writtenand verbal communication skills, and ADL and IADL performance. Assess cognitive, psychological and motor aspects ofperformance and develop individualized intervention plans toachieve favorable outcomes. Evaluate home environment for hazards or barriers to moreindependent living. Identify equipment needs and environmental adaptationsresulting in enhanced outcomes for clients. Continually assess and revise the occupational therapy careplan, and participate with nursing in the multidisciplinary careplan, as appropriate. Educate and instruct patients, family members, or other patientrepresentatives, in rehabilitative care and activities necessary topromote the client’s health, safety and independent living. Accurately document observations, interventions, andevaluations pertaining to patient care management and servicesprovided, utilizing a state-of-the-art touch pad tablet.

Occupational Therapist, OT Qualifications:

A current North Carolina Occupational Therapist license. Prior home care experience preferred, but not required A minimum of one year of recent work experience as anOccupational Therapist. Graduation from an occupational therapy program accreditedjointly by the Committee on Allied Health Education andAccreditation of the AMA and the AOTA, as indicated by schooltranscript or diploma. Ability to work independently and manage time effectively. Strong interpersonal skills, organizational skills, and problemsolving skills. Solid computer skills; prior experience with electronic medicalrecords (EMR) preferred.
