The OT will work with a dedicated team of homecare clinicians of RN’s, PT’s, Medical Social Workers, LNA’s and a long term Manager. Responsibilities:
Develops interventions to support goals of therapy and restore maximum function and strength promoting optimal health and minimal pain.
Evaluates results of occupational therapy by evaluating by observing, noting, and evaluating patient’s progress; recommends and implements adjustments as needed.
Determines occupational therapy treatment plans in consultation with physicians and homecare team.
Assures continuation of therapeutic plan following discharge designing and instructing patients, families, and caregivers in home exercise programs; recommending and providing assistive equipment.
Documents patient services in measurable goals.
Qualifications:
Knowledge of the principles, concepts and methodologies required of Occupational Therapy and it’s practical application in the treatment of patients at home
At least one year of experience in the hospital setting, facility or orthopedic center. Homecare experience preferred
Bachelor’s degree and NH Licensure as an Occupational Therapist
CPR certification
Valid drivers license, reliable vehicle and auto insurance
Demonstrated problem solving, empathetic communication and time management skills
EPIC (EMR) experience a plus!
Required:
Registered as Occupational Therapist with New Hampshire’s Board of Nursing.
CPR Certification.
Satisfactory criminal record.
