We are currently recruiting for a compassionate FullTime Home Health Occupational Therapist (OT) to reliably servepatients in the surrounding Plymouth Meeting, PA area. This is afull-time, pay per point position. you will:

• See a clientthrough their care needs
• from hospital discharge to living a safehome life with comfort, independence, and dignity.
• Make homevisits to clients in designated geographic territories.
• Performthorough evaluations of patients’ functional status, includingmuscle function, endurance, visual coordination, written and verbalcommunication skills, and ADL and IADL performance.
• Assesscognitive, psychological and motor aspects of performance anddevelop individualized intervention plans to achieve favorableoutcomes.
• Evaluate home environment for hazards or barriers tomore independent living.
• Identify equipment needs andenvironmental adaptations resulting in enhanced outcomes forclients.
• Continually assess and revise the occupational therapycare plan, and participate with nursing in the multidisciplinarycare plan, as appropriate.
• Educate and instruct patients, familymembers, or other patient representatives, in rehabilitative careand activities necessary to promote the client’s health, safety andindependent living.

What you should have:

• Active and currentlicense to practice as a professional Occupational Therapist (OT)in the state of PA.
• A minimum of one year of recent workexperience as an Occupational Therapist
• Previous home healthcareexperience welcomed, but not required. We will train you.
• Abilityto travel to cases as assigned
• A calm and reassuring presence,active listening skills, excellent assessment skills and strongwritten and verbal communication.
• The ability to deliver patientcare as you would want those you love to be treated, withcompassion, excellence, and reliability.
