Responsibilities:

Perform a thorough evaluation of dailyliving and work-related skills, assessing the needs of the personreceiving services and set measurable objectives that areformulated in conjunction with the rehabilitation team. Adjusttreatment as needed to achieve maximum resultsDesign communityreintegration activities to assist the person served in physicaland cognitive reconditioning and coordinate the plan with membersof the interdisciplinary team. Assist the person receiving servicesin understanding their needs and limitations and how to manage themduring functionRecommend and/or design special adaptive equipmentfor the person served to improve function in their home, communityand/or work environments. Incorporate the use of adaptive equipmentto enhance the rehabilitation programDesign retraining programs andtrain staff and family members to carry out the retraining programincluding dressing, feeding, grooming, hygiene and communityre-entry skillsProvide training to the person receiving servicesand as appropriate, all others involved in that person’srehabilitation. Analyze and document the result of anytrainingResponsible to apply professional knowledge and skills inaccordance with accepted standards of clinical practice, tofacilitate problem resolutions, and thereby the achievement ofindividualized goals and outcomes for the person servedReportprogress to the rehabilitation staff, person served and familymembers and observe/document and report changes in the condition ofthe person served to the Clinical Coordinator/designee, attendingphysician and other team treatment team members asappropriate.

Provide treatment progress and status documentation onthe person served in accordance with the Plan ofTreatmentParticipate in the team process provide input andeffective treatment strategies. Attend case conferences and familyeducational meetings.

Develop, teach and assess the plan oftreatment. Guide the Rehabilitation Specialist/Certified NursingAssistant in fulfillment of the plan. Use appropriate methods andtools in all teaching and training activities, including teachingin preparation for dischargePerform other duties as assigned inresponse to person served office and/or programneeds

Qualifications:

Bachelors degree in OccupationalTherapyMaintain professional license and/or certification asrequired by the state in which the therapist is practicing1 year ofexperience preferredKnowledge of rehabilitation techniques relatedto complex neurological injury generally requiredEffectivecommunication skills are also requiredCurrent CPR certification isrequiredThe candidate must successfully complete any/all requiredpre-employment evaluation test (s) per policy in each assigned areaof practiceGood organizational and communication skills are needed.
