Position Summary:

Theprimary function of your job is to return patient to optimalphysical function and facilitate the achievement of patients,physicians, and therapy goals within a SNF setting. Instruct thepatient in maintaining achieved level of function after dischargethrough education, independent exercise program, and appropriateactivity modification.

Education/Qualifications:

• Employee mustpossess a Bachelor’s Degree or Master’s Degree in OccupationalTherapy from an accredited college or university.
• Must possess acurrent and valid license granted from state of employment.
• Mustpossess basic computer skills and the ability to navigate throughvarious software programs, including service delivery software andthe electronic health record software.
