Staff BAYADA HomeHealth Care is currently seeking an experienced OccupationalTherapist, OT, for a full time opportunity performing home healthvisits for our South Burlington Visits office. This office servicesadult and geriatric patients on a per visit basis in homesthroughout Randolph, Topsham, and Chelsea . As a home careOccupational Therapist, you will be an integral member of amulti-disciplinary health care team that provides rehabilitativecare and skilled nursing to clients, affording them the opportunityto receive the medical care required to remain at home.

Occupational Therapist, OT Responsibilities:

• Make home visits toclients in designated geographic territories.
• Perform thoroughevaluations of patients’ functional status, including musclefunction, endurance, visual coordination, written and verbalcommunication skills, and ADL and IADL performance.
• Assesscognitive, psychological and motor aspects of performance anddevelop individualized intervention plans to achieve favorableoutcomes.
• Evaluate home environment for hazards or barriers tomore independent living.
• Identify equipment needs andenvironmental adaptations resulting in enhanced outcomes forclients.
• Continually assess and revise the occupational therapycare plan, and participate with nursing in the multidisciplinarycare plan, as appropriate.
• Educate and instruct patients, familymembers, or other patient representatives, in rehabilitative careand activities necessary to promote the client’s health, safety andindependent living.
• Accurately document observations,interventions, and evaluations pertaining to patient caremanagement and services provided, utilizing a state-of-the-arttouch pad tablet.

Occupational Therapist, OT Qualifications:

• Acurrent Vermont Occupational Therapist license.
• Prior home careexperience preferred, but not required
• A minimum of one year ofrecent work experience as an Occupational Therapist.
• Graduationfrom an occupational therapy program accredited jointly by theCommittee on Allied Health Education and Accreditation of the AMAand the AOTA, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Stronginterpersonal skills, organizational skills, and problem solvingskills.
• Solid computer skills; prior experience with electronicmedical records (EMR) preferred. BAYADA believes that our employeesare our greatest asset:
