MINIMUM QUALIFICATIONS

: High school diploma or equivalent. Effective communication skills, both written & verbal, organizational skills, knowledge of practical self-care skills & ability to work with minimal supervision. Ability to effectively work with persons with mental illness and/or developmental disabilities & implement rehabilitation / treatment / recovery plans for their program participation. Act 33/34/31 and FBI Clearances. Valid driver’s license and access to a vehicle during scheduled work hours.

POSITION SUMMARY

This position is responsible for assisting the Rehabilitation Coordinator and Therapist in providing the necessary support and information needed to implement each clients’ treatment plan. The incumbent also assists clients with daily activities, medication monitoring, management of daily tasks and providing transportation when needed. Crisis management and support is also an important function of this position.
