JOB REQUIREMENTS INCLUDE
: Screening of residents of various cognitive and medical diagnoses Knowledge and understanding of standardized assessments and evidence-based practice which are utilized to support clinical interventions. Provide skilled Occupational Therapy services to medically necessary residents with cognitive and functional disorders to improve, maintain or prevent further deterioration. Communication of resident findings and progress to the OT in order to make clinical adjustments and decisions based on individual resident needs. Monitor treatment plans for effectiveness to ensure residents’ functional outcomes. Provided education and consultation to residents, families, caregivers, and facility staff related to Occupational Therapy. Complete all necessary clinical documentation through each resident case. Communicates efficiently and effectively with Director of Rehabilitation, rehabilitation team members and other IDT staff regarding resident status, treatment plan, progress, barriers to progress and discharge planning.
QUALIFICATIONS/EXPERIENCE REQUIRES
Florida licensed/eligible in Occupational Therapy Assistant Knowledge of Net Health and Point Click Care Professionalism, excellent communication, and time management skills IND789
