Position Summary:

The primary function of your jobreturn patient to optimal physical function and facilitate theachievement of patient, physician, and therapy goals within a SNFsetting. Instruct the patient in maintaining achieved level offunction after discharge through education, independent exerciseprogram, and appropriate activity modification, under thesupervision of the OT and DOR.

Education/Qualifications:

• Employeemust hold an Associate’s Degree from an accredited college.
• Musthold a current and valid license granted from the state ofemployment.
• Current and valid OTA license granted from state ofemployment.
• Must possess basic computer skills and ability tonavigate through various software programs, including servicedelivery software and the electronic health record software.
