1099 Contract — Ophthalmologist or Optometrist We are seeking to contract an Optometrist in Augusta, GA. -to helpensure our service members get the best care possible. QTC Medical Group ofAlaska:
As a contracted Ophthalmologist or Optometrist with Jamshid Tamiry,M.D., P.C., doing business as QTC Medical Group of Alaska, anaffiliate of Leidos QTC Health Service, you will be responsible forperforming a range of duties including: Performing examinations Interpreting clinical data Completing written reports and generating independent medicalopinions (IMO) The examinations must be conducted utilizing the Kinetic PerimetryVFT Equipment Compatibility Chart.
Primary Responsibilities:
Conduct exams for a variety of customers to include federal,defense and others. Complete independent medical opinions regarding eyeconditions.
Perimetry requirements:
Results must chart 16 meridians, 22.5degrees apart for each eye and indicate the Goldman equivalentused. Diagnostic testing
Required Qualifications:
Education:
Ophthalmologist (M.D.) or Optometrist (O.D.) from an accrediteduniversity or college Completed Education and training in an US accredited college oruniversity –
Experience:
Minimum 2 years of post-graduate experience as an
Ophthalmologist or Optometrist Certification:
Unrestricted State medical licensure withoutlimitations to perform full scope of authorized practice
Clearance:
Ability to pass a medical clearance Must be able to obtain and maintain government clearances(LARS, Public Trust, NACI, BGI, etc.) as required.
Preferred Qualifications:
Experience conducting occupational/disability medicalexaminations

Appointments are typically scheduledapproximately 5 to 7 days of notification. You set the pace andfrequency!

Compensation:
Compensation will be provided for eachexamination, with payment determined based on the specificrequirements and type of examination performed.
Additional Information:
Provider must complete required online training courses, up to9 courses at approx. 12.5 hours total. Leidos QTC Health Services will run a 7-year criminal historyand sex offender background check. Once the BGI and credentialing have been completed, theProvider will be required to obtain CPEP Certification (approx.17hours/depending on specialty) You will be given access to the Provider Liaison (PL)department; this department assists with any issues, questions, orneeds you may have while contracted with Leidos QTC HealthServices. –
