Overview:
Provides a coordinated, strategic approach to detect early and manage effectively the chronically ill patient population. Collaborates with inpatient case management as appropriate. Implements an effective internal tracking system for identified patients. Coaches patients/families toward successful self-management of their chronic disease.
Qualifications:
Education Required:
Associate’s in Nursing License, Certification and/or
Registration:
Alabama Registered Nursing License.
BLS Certification Experience:
3 years nursing experience
