Chronic Care ManagerJob Description The Chronic Care Manager plays a vital role in providing one-on-one attention via telephone to patients with multiple chronic diagnoses. This position involves collecting patient health information to identify risks, providing education, and encouraging compliance with treatment plans approved by healthcare providers. The Care Manager is responsible for creating personalized Care Plans with goals and interventions to support and monitor patient needs, as defined by the primary care physician (PCP). The Chronic Care Management (CCM) program aims to promote the highest level of quality care for patients. Responsibilities + Conduct initial risk assessments to gather clinical elements necessary for developing patient-specific Care Plans, approved by providers. + Collaborate with patients to plan and monitor their care as determined by their PCP. + Encourage compliance with PCP and specialist office visits. + Promote adherence to medication regimens. + Serve as a patient advocate, liaison, and information resource. + Educate patients on managing chronic and complex conditions. Essential Skills + Licensed Practical Nurse (LPN) required. + Proficient in Microsoft Outlook and Excel. + Familiarity with documentation in Electronic Medical Records (EMR). + Must possess high-speed internet connection (not satellite or hotspot). + Self-motivated and capable of working in a remote environment. + Passionate about helping others. Additional Skills & Qualifications + Experience in chronic care management. + Strong communication and interpersonal skills. + Ability to work independently and manage multiple tasks efficiently.
