JOB SUMMARY:
The primary function of the Specialty Care Navigator is to ensure effective facilitation of clinic activity, patient recruitment, and services regarding Pre-Exposure Prophylaxis as a vital method of HIV prevention. The Navigator will increase the volume of patients receiving PrEP by initiating contact with high-risk individuals; provide direct linkage to care for people seeking access to PrEP, exercise cultural competency amongst various demographics and sub-populations. All services provided for PrEP patients will be done so in a non-judgmental and orderly manner
DUTIES AND RESPONSIBILITIES
1. Promote and increase the awareness of PrEP among Franklin employees and the community within the organization’s service are. 2. Provide basic clinical and non-clinical linkage to care and patient navigation for individuals seeking PrEP as a practical HIV prevention tool 3. Provide educational in-services to various departments and programs within the organization. 4. Work with patients to assess needs, develop care plans and coordinate PrEP related services through advocacy, education, support, empowerment and coaching. 5. Serve as the liaison between the medical providers and patients. Ensure patients are following orders in a timely manner. 6. Link and provide patients with resources to fully access PrEP services with little to no barriers. This will include but not limited to, completing forms to obtain medication assistance directly from pharmaceutical companies and obtain approvals to various third-party payers. 7. Interface with clinical and non-clinical staff members. This will include individuals from other entities such as pharmaceutical representatives. 8. Educate patients on safer sex practices. 9. Expand the programs’ printed and online educational material on PrEP services. 10. Provide HIV Counseling, Testing and Referrals as needed to at-risk populations in compliance the standards of the Alabama Department of Public Health. 11. Maintain accurate and complete participant records and reports, tracking contact, participants, outreach, counseling, testing and referrals. 12. Cross train in other areas of specialty, Hepatitis C and HIV and assist in these areas when needed.
QUALIFICATIONS
EDUCATION
AND EXPERIENCE
The preferred qualification for the Specialty Care Navigator position is a candidate who licensed to practice as an LPN in the State of Alabama, or holds a Bachelor’s of Science in the areas of human services (Social Work, Sociology, or Psychology) with a strong science background. One year of experience in HIV and STI prevention, healthcare systems, and health promotion and six months of experience working with an electronic health record system.
KNOWLEDGE, SKILLS & ABILITIES
Must be comfortable handling and discussing issues related to HIV/AIDS, intimacy, and alternative lifestyles. Possess the ability to work within a multi-disciplinary team approach to health care and uphold federal and local standards of patient confidentiality.

Valid Alabama Driver License.
