We currently have an exceptional career opportunity for a Central Access Navigator to join our team in Norwich, CT to manage referrals and bed availability for identified SCADD programs ensuring that individuals seeking treatment receive proper screening and referrals to appropriate levels of care.
Responsibilities include:
Manages all referrals for admission and networks with referral sources for proper treatment placement and planning. Completes insurance verification and identifies payment resources prior to admission, as well as assesses client eligibility for benefits. Fields referrals from all sources and provides information according to SCADD protocols. Responds to referral calls, emails, website and social media inquiries as well as other points of origin and directs them appropriately. Provides brief screenings that encompass ASAM criteria, presenting problems in substance abuse and/or mental health as well as any extenuating medical issues. Consults with proper SCADD medical or clinical personnel for complex referrals. Fields crisis calls to respond urgently or emergently as needed. Assists with arranging transportation when needed. As deemed necessary per program, ensures required authorizations from appropriate payors are secured and documented in the electronic medical record. Coordinate and track admissions to maximize bed capacity utilization. Attend program team meetings as necessary to maintain productive communication Compiles data on referrals, occupancy and daily census for DMHAS and Administration Office. Maintain networking relationships with referral sources to sustain capacity across the organization. Will require weekend shifts.
