Responsibilities The Patient Navigator Lead assumes all responsibilities of the Patient Navigator role and in addition is responsible for overall coordination of activities related to the Patient Navigator Team and Bundle Payment Initiatives. The Patient Navigator Lead is accountable for leading the collaboration between the Patient Navigators and our Post-Acute Care Partners. The Lead has oversight of the daily operations of the team in regards to Readmissions efforts and ensuring coverage across both campuses to attain readmission goals. The Patient Navigator Lead has direct responsibility for episode management of bundle patients and understanding goals of BPCI program. The Patient Navigator Lead maintains weekly performance tracking and communication to VP and PACC as appropriate. Identifies and implements best practices for operations and sharing of knowledge to enhance service and quality of the organization. Develops appropriate education and communication plans with PACC partners to improve patient outcomes and meet bundle initiatives. Partners with our physicians directly involved in our bundle payment and readmission processes to improve efficiency, meet bundle requirements and improve outcomes. Facilitates problem resolution, process improvement and communication flow. Qualifications Education
Bachelor’s Degree (required)
Master’s Degree (preferred)
Licensure/Certification/Registration
Florida Registered Nurse License (Preferred)
BLS
