Targeted Case Manager ( Home Health) PFMDINC Home Health Agency Miami Shores, FL 33153 Job Summary We are seeking a dedicated and compassionate Case Manager to join our healthcare team. The Case Manager will play a crucial role in coordinating patient care, ensuring that individuals receive the necessary services and support throughout their treatment journey. This position requires a strong understanding of patient care, medical terminology, and the ability to work collaboratively with healthcare professionals in various settings, including ICU, acute care, and clinics. The ideal candidate will have experience in addiction counseling and managed care, providing comprehensive support to patients and their families. Duties Assess patient needs and develop individualized care plans that address their medical, emotional, and social requirements. Coordinate with healthcare providers to ensure seamless patient service across different departments such as hospital medicine and outpatient clinics. Monitor patient progress and adjust care plans as necessary to meet evolving needs. Provide education and resources to patients regarding their conditions, treatment options, and available support services. Facilitate communication between patients, families, and healthcare teams to promote understanding and collaboration. Document all interactions with patients accurately in accordance with medical terminology standards. Advocate for patients’ rights and needs within the healthcare system. Participate in case conferences and multidisciplinary team meetings to discuss complex cases. Requirements Bachelor’s degree in Nursing, Social Work, or a related field; relevant certifications are a plus. Proven experience in patient care settings such as ICU or acute care environments. Familiarity with addiction counseling practices is highly desirable. Strong knowledge of managed care principles and medical terminology. Excellent communication skills with the ability to empathize with patients from diverse backgrounds. Strong organizational skills with attention to detail in documentation and case management processes.
