DUTIES AND RESPONSIBILITIES

: Enhances accessibility and communication across all Caron programming to make it possible for creative treatment planning and services to potential families, patients, and referrals. Assists in providing concierge service to patients placed into Caron’s continuums and assists with case management needs for family members and referrals; this includes efforts to solidify/confirm potential admissions, as well as provide the services throughout a treatment episode including continuing care planning with an emphasis on patients in Signature programing. Ensures that all communication with referrals is tracked in Salesforce/Aura. Collaborate closely with the clinical team to provide ongoing case management and ensure each patient’s care plan is executed effectively. Attends and actively participates in Signature treatment team meetings to discuss patient progress, clinical updates, care coordination and provides any relevant updates related to referrals, industry changes and census trends. Works with Corporate Director of Admissions, Regional Resource Directors team, and others to manage and effectively respond to admissions and census needs across the continuum. Works collaboratively with leadership to assure Caron’s Core Values and Mission Statement is being presented consistently to the patients, their families, and referrals. Collaborates with Admission Leadership and clinical directors regarding patient placement within or outside the continuum based on clinical assessment and medical records as appropriate. conversions to full programming Provides clinical assessment and interpretation as needed for all Signature patients. Provides a sophisticated level of financial conversation to include vetting all appropriate resources for treatment investment, provide details of inclusions and exclusions of treatment investment, utilization of all financial options. Proficiency in verification of benefits Responsible for all financial follow-up- treatment payment, pharmacy set up, labs, and any identified follow-up Provides onsite support to help facilitate concierge admissions as well as physical admissions as necessary Ensures all appropriate admissions protocols are utilized with key stakeholders and ensures appropriate ongoing communication and documentation throughout the remainder of the treatment episode. Aids throughout treatment episode that includes continuing care planning with an emphasis on Caron First Signature programming. EXPERIENCE / EDUCATION QUALIFICATIONS

Minimum of 3 years’ experience working in a clinical setting within a substance use disorder. Minimum bachelor’s degree required or master’s preferred. Minimum of 2 years’ case management experience. If in recovery, 2 years of continuous sobriety is preferred.

Must be able and willing to travel as needed. Must be able to deliver presentations to referrals that may be on campus and at their strategic geographic locations. Ability to provide tours of our campus, which may include ambulating throughout hilly areas and ascending and descending stairs. This position may allow for remote work and therefore requires a secure office space with high-speed internet.

KNOWLEDGE, SKILLS, ABILITIES

An extensive background in clinical programming of all Caron continuums. Must demonstrate strong communication, interpersonal and organizational skills. Ability to schedule and coordinate site visits. Must possess skill with Microsoft Office products. Ability to establish and maintain effective working relationships with employees and heads of the various departments. Ability to develop, re-establish, and maintain relationships with referrals and our own Business Development Officers (BDO) across the country.
