Job Description

Healthcare Navigator for Veterans (Broward & Miami Dade County) 3.0 3.0 out of 5 stars South Florida, FL 33082 Advocate Program, Inc. is a multi-faceted community agency whose mission is to facilitate social change and enhance public safety through leadership, research, education, supervision, and services, utilizing best and evidence-based practices. Supportive Services for Veteran Families(SSVF) Health Care Navigators are employed by SSVF grantees to provide services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. SSVF health care navigators s provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties. SSVF Healthcare Navigators work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team. The HealthCare Navigator (HCN) Specialist will support homeless veteran families which includes connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. SSVF health care navigators provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties. SSVF Healthcare Navigators work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team.

Responsibilities:

1. Works closely with Veterans to assist them in communicating their preferences in care and personal health-related goals to facilitate shared decision making of the Veteran’s care. 2. Serves as a resource for education and support for Veterans and families and helps identify appropriate and credible resources and support tailored to the needs and desires of the Veteran. 3. Monitors Veteran’s progress, maintains comprehensive documentation, and provides information to treatment team members when appropriate. 4. Assists Veterans in identifying concerns or questions about their treatment or medications to develop open communication with the provider or treatment team. 5. Acts as a health coach by proactively supporting the Veteran to optimize treatment interventions and outcomes. 6. Modifies services to meet the needs of Veterans best and coordinates services with other organizations and programs to assure such services are complementary and comprehensive; directs activities to maximize effectiveness, efficiency, and continuity of care for Veterans; provides case management services to Veterans serves as the liaison to VA and community health care programs, and represents the program in contacts with other agencies and the public. 7. Helps coordinate supportive and additional services with the Veteran 8. Coordinates referrals to VA, community health clinics, and other programs needed to ensure access to health care. 9. Advocates for Veterans’ needs. 10. Regularly consults with other team members and appropriately assesses and addresses the needs of the Veteran. 11. Assists in developing policy, procedures, and practice guidelines related to the specialty program using knowledge gained from research or best practices. 12. Develops relationships with community leaders, VA staff, and other referral networks. 13. Develops evaluation components and outcomes indicators and report those evaluation results to VA and organizational leadership. 14. Travels within Broward, Miami Dade and Monroe County. Mileage is reimbursed. 15. Maintain timely and appropriate documentation, including progress notes to be provided to Case Managers/ Care Coordinator on a weekly basis. 16. Provides services designed to enhance client health and independent living skills, thereby promoting client housing stability. 17. Completes all documents and data entry required of the job accurately and in a timely fashion. 18. Participates in case management meetings to discuss progress with cases referred for housing placement. 19. Develops a trusting and working relationship with the individual/ family seeking help. 20. Works as part of a multi-disciplinary team providing “client centered services” with all staff members, as well as the other case managers participating in the program. 21. Reports to the supervisor on all issues relevant to program’s functioning, including the interagency referral process. 22. Participates in in-service training as needed to develop and/or enhance the knowledge, skills. 23. Maintains all required documentation in participants’ confidential case records, and assisting the Program Manager and Director with the preparation of any required program and statistical reports. 24. Accepts and respond to supervision by the Program Director. 25. Performs other duties upon request.

OUTREACH/ INTERPERSONAL SKILLS AND LEADERSHIP SKILLS

The ideal candidate is an individual with experience working in teams and possessing team leadership skills; someone who is self motivated and able to motivate others, who enjoys social interaction and engagement, who can successfully gain the acceptance and trust of clients, as well as with staff in other agencies that have entered into referral and service coordination agreements with the Advocate Program, and possess strong work ethic and organizational skills. EDUCATION and/or

EXPERIENCE

Bachelor’s level social worker or equivalent education and 3 years of experience is preferred.

COMMUNICATIONS AND INTERPERSONAL SKILLS

Must have excellent interpersonal skills coupled with excellent oral and written communication skills. Must possess strong observation and analytical skills; functional knowledge of emergency assistance services in Miami Dade County; strong personal responsibility. Flexible approach to working hours is mandatory, as it impacts ability to attend outreach events, and the ability to meet with participants after normal business hours.

LANGUAGE SKILLS

Ability to read, analyze and interpret program procedures, outreach protocols and related policies. Ability to write reports, correspondence, and assist team in outreach tools development. Ability to effectively present information and respond to questions about the Program from groups of managers, clients, customers, and the general public.

TECHNICAL SKILLS

Must have demonstrated skills in the MS Office 2000 Suite or higher (includes

WORD, EXCEL, POWERPOINT, EMAIL

OUTLOOK or

EMAIL EXCHANGE

) and Internet Explorer. Ability to use or is familiar with web based social networking tools such as Facebook, Twitter, etc. Additional knowledge in the following is ideal, but not required: Adobe Acrobat for PDF creation; Document Scanning.

CERTIFICATES, LICENSES, REGISTRATIONS

Must possess a valid Florida driver’s license.
