Schedule:

Full-time, Mon – Fri, 8:15am-5:00pm Occasional evening/weekend flexibility may be required for community events.

What You’ll Do:

As a Case Navigator, you’ll be a key member of our multi-disciplinary outpatient behavioral health team. You’ll work closely with nursing staff, physician extenders, and psychiatrists to support whole-person care with a focus on healthcare coordination. You’ll help clients access vital primary and specialty healthcare services, track key health indicators, and provide direct support to empower them in managing their mental and physical health.

Key Responsibilities:

Participate in interdisciplinary treatment team meetings and share observations to support individualized care planning Collect client health data (vitals, histories, screening tools) and assist in monitoring wellness indicators Provide ongoing support and medical case management based on clients’ treatment plans Build strong relationships with healthcare providers to facilitate coordination of care Deliver individual skills training to help clients manage symptoms and enhance daily functioning Ensure accurate and timely documentation of all service interactions Maintain regular outreach to clients and collaborate with internal teams to overcome barriers to care Prioritize ethical standards and communicate client concerns or safety issues promptly

What You Bring:

Bachelor’s degree in a health-related field OR completion of an accredited Medical Assistant program Certification by AAMA, ARMA, NCCT, NHA, AMT, or AAH is a plus One or more years of experience in a healthcare setting Strong technology skills, including EMR systems, telehealth tools, and communication platforms Excellent interpersonal and communication skills Ability to work independently and as part of a team Reliable transportation and willingness to work in a community setting Why Join Us?
